Read Japanese?

Under the cut is a large image of what I think is a packing slip from a microscope. If you read Japanese, could you give me an idea if I'm right about that, and if so, can you tell when it's from? Is that stamped "41.8.18" a date in 1941?

I clicked through out of idle curiosity, and I can tell you it is for a microscope. Looks like a warranty not just a packing slip. The date is Showa 41, so 1966. I'm guessing that's the date it was quality inspected. There's a spot for the date of purchase just above the box, but it's blank.
