It's my understanding that the State of California requires insurance policies that cover other prescription drugs to also cover all FDA-approved contraceptive drugs and devices, as well as related medical services. See link

Can you let me know the basis on which you deny coverage for the IUD, which is an FDA-approved contraceptive?

When I got mine, my Dr. wrote me a perscription for the device, which I went to the pharmacy and dropped off. They fufilled the perscription by supplying the device for my co-pay ($10). I brought the sterile wrapped IUD upstairs to the Dr.'s office, and they inserted it, covered by my insurance.

Now, this was all at Harvard, which has amazing insurance, so.....

Still, something like that may be a work around.
