It's my understanding that the State of California requires insurance policies that cover other prescription drugs to also cover all FDA-approved contraceptive drugs and devices, as well as related medical services. See link
Can you let me know the basis on which you deny coverage for the IUD, which is an FDA-approved contraceptive?
